  • Start date ASAP - 10/7
  • Department: Pharmacy - Pharmacist
  • Emergency medication protocol: ACLS - preferred attendance at codes
  • Experience with Vancomycin AUC dosing required
  • Familiarity with EPIC, Lexicomp, Baxter IQ essential
  • Day and Evening shifts needed, current day availability
  • Weekend requirements: every other or one per month preferred
  • 2/3 holidays required
  • Medication Dispensing System experience: Omnicell, Central pharm robot, carousel, Omnicell XT cabinets, and Omnicell Controlled Substance cabinet
  • Dress Code: Business casual/white coat or Khaki scrubs
  • Orientation including one NEO, one clinical ed/EPIC, one day floor
  • Schedule: 7 on 7 off, 10 hour days, Thursday - Wednesday, evening shifts as needed
    • Job Requirements
    • Must have 2 years of true hospital pharmacy experience of like size (250 census)
    • License: Active AZ or Compact license required
    • Resume must include detailed duties, hospital size, unit size, and trauma level (if applicable) for each hospital
    • Explanation of any employment gaps required upon submission
      • Schedule Information
      • 7 on 7 off, 10 hour days, Thursday - Wednesday
      • Evening shifts as needed
      • One clinical manager on-site 5 days a week
      • Day and Evening shifts current availability required
      • Weekend requirements: every other or one per month preferred
      • Holiday Requirements: 2/3 holiday required

        • Department-specific orientation: One NEO, one clinical ed/EPIC, one day floor
        • Unit/Department: Pharmacy
          • Additional Details
          • ACLS preferred - will attend codes
          • Preference for IV pharmacist/compounding pharmacist
          • EPIC experience required with no inconsistencies on resume versus cover page
          • EMR listed on each work assignment
          • Time off requests limited to 5 days or less and included within the attached profile/resume
