Pharmacist Location: Yuma, AZ Agency: Mindlance Health Pay: $1,672 per week Shift Information: Days - 4 days x 10 hours Contract Duration: 13 Weeks Start Date: 2/9/2026 About the Position ID: 74616948 Shift: 8H Evening, Night 7 on 7 off Shift 1300 - 2330 Description: Unit Date: 2/9Department:Pharmacy | Pharmacy Tech*Certifications: Active AZ- Pharmacy Tech license*Radius Rule: 100 milesStart dates are not flexible.SUBMISSION PROFILE REQUIREMENTS - Must be in the profile or cannot move forward with submittal. Start Date Must match the next available start date, include in attached profile/resume EPIC experience required. Ensure it is accurate (there have been inconsistencies on the resume versus the cover page) EMR listed on each work assignment Time Off- Ensure dates make sense for the required start date and are 5 days or less. Include within the attached profile/resume. All RTO needs to be in at time of submission. Resume- Ensure resume information is accurate (there s been an increase in inaccurate information sent). MUST INCLUDE: detailed duties, hospital size, unit size, and trauma level (if applicable) for each hospital. Explain any employment gaps upon submission and include in attached profile/resume. Certifications- Make sure certs are accurate and up to date.License- Active and in hand - AZ or Compact- Ensure it's accurate- Certs/Licensure Requirements: AZ-active pharm tech license and active PTCB certification. - What areas will they cover?: All areas- Sterile compounding and hazardous compounding are a bonus. IV room experience is also a bonus. Automation is also very important.- Common Duties/Job Requirements/Skills: Delivery of meds, restocking of medications.- Experience: Pyxis or Omnicell experience. EPIC experience preferred.- EMR: EPIC- Is there a Pharmacy Manager/Supervisor on each shift?: One clinical manager on 5 days a week.- Do you staff Pharmacy Techs?: Yes- Shifts & Scheduling: Day and Evening needs.- Weekend Requirements: Every other or one per month preferred.- Holiday Requirements: 2/3 holiday required.- Medication Dispensing System: Omnicell, Central pharm robot and carousel, Omnicell XT cabinets, Omnicell Controlled Substance cabinet.- Scrubs or Dress Code Requirements: Khaki scrubs.- How many shifts of unit/department-specific orientation: One NEO, one clinical ed/EPIC, one day floor. Cancellation may cancel hours equivalent to the weekly guaranteed hours for the assignment 36, 40, or 48 for a 13 week assignment. For all 26-week assignments, the cancellation policy will be doubled. Charting BSN License to Trauma Level:2 Care Shift Notes:7 on 7 off Shift 1300 - 2330 Required is not required, Certified Pharmacy Technician (CPhT) via PTCB Verification Guaranteed Hours: 72 hours per two weeks Contract Weeks:91 Requirements Required for Onboarding BLS Core Exam - Allied/Non-Nursing Ishihara Colorvision Screening Pharmacist Exam Pharmacist Skills Checklist
