Physical Therapist Location: Yuma, AZ Agency: Cynet Health Pay: $2,586 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position Job Title: Physical Therapist Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand, no exceptions Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: This position involves making home visits for physical therapy. Periodic office meetings are required. Reliable transportation, a current driver’s license, and agency-required liability insurance are necessary. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points. No guaranteed hours. If the contractor is unable to meet financial needs due to low patient census, they should reach out to the Account Manager to discuss floating to a nearby clinic for additional hours. Types of cases include Start of Care and Follow Ups. Candidates must confirm they are aware this is a home health assignment and that they will be responsible for driving to patients' homes daily. Clinicians must also confirm they have a vehicle, driver’s license, and auto insurance. The mileage radius for this position is 50-75 miles, which may result in longer drives between patients. All documentation must be completed within 24 hours of the appointment. Delayed or incomplete documentation that impacts billing will result in a cancellation. Contractors are expected to hit weekly productivity points of 30, which equates to 40 hours of work per week. Substituting or declining patients may result in cancellations. All overtime must be pre-approved. Travel up to 75 miles from the assigned care center is required. Local candidates are accepted.
